Organisation designed for wellbeing at home

Upon receipt of a request, a coordinator contacts the beneficiary or their carer to arrange a meeting and assess their needs.

In collaboration with the beneficiary, their family and friends, and their attending physician, a support and care plan is proposed.

The coordinator works with all psycho-medical-social actors, without exclusivity or obligation. Each beneficiary is therefore free to choose their service providers.

With a comprehensive overview of the situation and knowledge of the psycho-medical-social sector, the coordinator ensures the implementation of a personalised, multidisciplinary and evolving care and support plan.
